Some of the most natural aesthetic results we produce at our Encino practice come from a treatment that shows almost nothing on the day it is performed. Biostimulators are poly-L-lactic acid treatments: they do not fill, it instructs. Over three to six months, your own fibroblasts lay down new collagen in response to the product, gradually restoring facial volume and skin thickness in a way that is very difficult to identify as treatment.

The Mechanism

Poly-L-lactic acid is a biocompatible, biodegradable synthetic polymer used in absorbable sutures for decades. Injected into the deep dermis and subcutaneous plane, its microparticles trigger a controlled, subclinical inflammatory response. Fibroblasts migrate to the area and deposit new type I collagen around the particles. The PLLA itself is fully metabolized into carbon dioxide and water within months — what remains is your own tissue. That distinction matters: with hyaluronic acid you are seeing product, with biostimulators you are seeing collagen.

Who Benefits Most

Biostimulators excel for diffuse, global volume loss rather than a single hollow. The classic candidate is a patient in their forties, fifties or sixties whose entire face has thinned — flattened temples, deflated midface, crepey texture, a generally drawn appearance. It is also excellent for patients who have lost significant weight, including the increasingly common presentation after GLP-1 therapy, where facial fat loss outpaces the rest of the body. Many patients also treat the décolletage, buttocks and upper arms, where skin quality and mild laxity respond well.

Biostimulators are a poor choice for lips, and for patients who want an immediate result before an event next week. It is also not the right tool for sharply defined structural projection — a chin or a cheekbone edge is a filler job.

The Protocol

The biostimulator arrives as a lyophilized powder that must be reconstituted with sterile water and, in our protocol, lidocaine. Proper reconstitution and adequate hydration time are non-negotiable — the majority of nodule complications historically traced back to inadequate dilution and poor technique, not the product itself. We inject in the deep plane using cannula and needle depending on region, distributing evenly rather than depositing boluses.

  • Most patients need two to four sessions, spaced four to six weeks apart.
  • A general guideline is one vial per decade of life, adjusted for degree of volume loss.
  • You will look mildly swollen and full on the day of treatment — that is fluid, and it resolves within two to three days.
  • Real change becomes visible around week six and continues through month six.
  • Results typically last two years or longer, with many patients maintaining with a single vial annually.

The Five-Five-Five Rule

We ask every biostimulator patient to massage the treated areas five minutes, five times a day, for five days. This distributes the particles evenly through the tissue and is the single most effective step in preventing palpable nodules. Patients who follow it reliably have very few issues; patients who skip it are the ones who call us later. We demonstrate the technique before you leave.

Recovery And Side Effects

Expect tenderness, mild swelling and possible bruising for two to five days. Avoid strenuous exercise and heat exposure for twenty-four hours. Small subcutaneous papules can occur weeks to months later; most are self-limited and many respond to massage or dilute steroid. Because PLLA is not reversible with hyaluronidase, careful patient selection, conservative dosing and correct technique matter enormously — we would rather add a fourth session than over-treat in the second.

They solve different problems and frequently work together. Biostimulators rebuild the broad foundation and improves skin thickness across the face; hyaluronic acid filler then defines specific structures with precision. In a typical Encino patient over fifty, we might complete a biostimulator series first, wait three months for collagen maturation, and only then decide how much filler — often far less than originally anticipated — is genuinely needed. Sequencing this way costs less over time and looks better.
